Nurse-Family Partnership (NFP) is an evidence-based, community health program with 45 years of research showing significant improvements in the health and lives of moms and their children affected by social and economic inequality.

Nurse-Family Partnership’s mission is to positively transform the lives of vulnerable babies, mothers, and families.

NFP’s vision is a future where all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken.

The Western Wisconsin Nurse-Family Partnership Consortium (WWNFPC) is a group of local health departments that work and collaborate in achieving NFP’s goals. 

Marathon County Health Department is a member of this consortium along with Chippewa County Public Health, Dunn County Health Department, and Eau Claire City-County Health Department.

If you or the person you are referring lives in Chippewa, Dunn, Eau Claire, or Marathon County, then you can sign up with us! If you don’t reside in any of these counties, then you won’t be able to sign up with our Consortium.
